The forms below will be used as verification that you have successfully completed the PWL Online Educator Training. Once these forms have been processed you will receive an official Places We Live PLT secondary module guide, Certificate of Completion, and a receipt for your payment.
Directions for submitting the documentation are included in the checklist file. Documents are provided as MS Word and PDF files. Please keep a copy for your records.
